Moreover, the boundary between the patient being ?there? or ?not there? is only getting hazier as science progresses. In the end, even physicians don?t always seem to understand the power they may be wielding over life and death.

Planning for your end-of-life scenarios is never easy, nor is it easy to faithfully interpret the wishes of a loved one. Nevertheless, as with all aspects of estate planning, communication with your agents, loved ones, physician and spiritual advisor holds the key to success as you define it.

Protest as Iraq moves to close 44 media outlets

BAGHDAD (AP) ? An Iraqi press freedom group is condemning the government for ordering the closure of 44 news organizations, including a U.S.-funded broadcaster.

Ziyad al-Aajely, head of the Journalistic Freedoms Observatory, calls the move "a setback to the freedom of journalism in Iraq."

The Iraqi media commission's Safaa Rabie denies any crackdown, saying Sunday it only seeks to close offices without an operating license. Some foreign media on the list no longer keep news bureaus in Iraq.

However, one broadcaster targeted for shutdown, U.S.-funded Radio Sawa, says it does have a license. Radio Sawa deputy director Salah Nasrawi says he was surprised to see the station on the list but said it could be a bureaucratic error.

Critics have accused Iraqi Prime Minister Nouri al-Maliki of muzzling the press to consolidate power.

Grb2 holds powerful molecular signaling pathway in check

Once considered merely a passive link between proteins that matter, Grb2 - pronounced "grab2" - actually lives up to its nickname with its controlling grip on an important cell signaling pathway, scientists at The University of Texas MD Anderson Cancer Center report in the June 22 issue of Cell.

"Grb2 is a switch that controls normal signaling through the fibroblast growth factor receptor (FGFR)," said the paper's senior author, John Ladbury, Ph.D., professor in MD Anderson's Department of Biochemistry and Molecular Biology.

"Perhaps the best way to think about it is that Grb2 controls cell homeostasis (stable state) before a growth factor binds to FGFR, activating this molecular pathway," Ladbury said.

In addition to discovering a fundamental aspect of FGFR signaling, the researchers' discovery points to a potential explanation of why genomic alterations found in breast, bladder and gastric cancers and melanoma might promote cancer formation and growth, Ladbury noted.

FGFR has a docking station to receive growth factors on the cell surface, and another internal region that passes the growth factor signal on to proteins inside the cell by attaching phosphate groups to them.

FGFR employs phosphorylation to regulate a number of important processes, including the cell cycle, cell proliferation and migration. When some of these pathways become overactive, they can contribute to cancer growth and survival.

Like "a car idling in neutral" ready to go

Grb2's full name reflects its location: growth factor receptor-bound protein 2. In the great rush of molecular signaling pathway mapping in the 1990s, Ladbury noted that Grb2 was labeled an "adaptor protein," one that has no activity of its own apart from connecting to other proteins.

Mapping ran way ahead of figuring out each protein's function in a signaling pathway, Ladbury said, and scientists are still catching up in that area.

"When you think about it, why would a cell bother to produce a protein that plays only a passive role linking one protein to another?" Ladbury said. He and his colleagues found that's simply not the case with Grb2.

They demonstrated that Grb2 binds to the internal signaling region of FGFR, preventing the receptor from activating other pathways while at the same time allowing a baseline level of phosphorylation of FGFR that isn't strong enough to initiate a signal by recruiting other proteins. This baseline phosphorylation occurs without a growth factor activating the receptor and only happens if Grb2 is bound to FGFR.

"You can think of this like a car that's idling in neutral," Ladbury said. "Its engine is running but it's not going anywhere. "

But it is primed for action, and the team found that this idling version of FGFR is more likely to attract an external growth factor that triggers full signaling. They also found when the growth factor FGF docks at the receptor and activates it:

FGFR then attaches a phosphate group to the Grb2 that was holding it in check.
Phosphorylated Grb2 disconnects from the receptor.
With its internal signaling domain now clear of Grb2, FGFR can change the shape of the domain so it can signal to other proteins by phosphorylating them.

"The growth factor essentially slots that idling car into gear, and off it goes," Ladbury said. Having FGFR warming up before activation appears to be a more efficient way for a cell to activate the pathway than starting from a state of zero phosphorylation.

There's reason to believe this mechanism may apply to other tyrosine kinase receptor proteins, a class of receptors including FGFR that activate signaling cascades by phosphorylating other proteins, Ladbury noted. Additional research will be required to sort out that possibility.

Potential role in cancer suppression

Their findings provide a possible mechanism to explain why genomic deletions found in bladder and gastric cancer and point mutations in melanoma might promote those cancers.

In each case, the portion of the gene that encodes FGFR's internal signaling domain is affected. If that domain is abnormal, Grb2 would not be able to bind it."That could lead to the FGFR pathway being turned on constantly," Ladbury said.

FGFR launches the MAPK signaling pathway, which is known to promote cancer when abnormally activated. Ladbury and colleagues have identified a potential tumor-suppressing role for Grb2.

The team developed and confirmed the relationship between Grb2 and FGFR via cell biology experiments, biophysics and structural analysis."I'm extremely proud of this group, which can cover all of those bases and provide a full explanation of a system," Ladbury noted.

The Ladbury group continues to investigate:

Whether cancer cells are predisposed to have abnormal levels of Grb2 , which would affect the control of FGFR signaling.
How disruption of the cycle of Grb2 binding and release leads to cancer.
The mechanism by which Grb2 is phosphorylated by FGFR.

Asteroid venture looks for a few good ideas

Planetary Resources, the billionaire-backed private venture that's aiming to hunt down and mine near-Earth asteroids, is looking for suggestions about projects that could attract extra funding through Kickstarter-style campaigns.

In a Web posting, co-founder Peter Diamandis says that in the month and a half since the asteroid-mining project was unveiled, he and his colleagues "have been overwhelmed at the response from people begging to know how they can get involved." In an associated email blast, Diamandis and the company's other co-founder, Eric Anderson, say they've gotten hundreds of emails asking about the project.

The company ??which counts Google billionaires Larry Page and Eric Schmidt, space billionaire Charles Simonyi and Texas billionaire Ross Perot Jr. among its investors ? has said it plans to launch its first asteroid-hunting space telescope, the Arkyd-100, within two years. But what Diamandis and Anderson really want to do is launch 10 to 15 of the 44-pound (20-kilogram) telescopes in the next three years.?

"To offer you?a chance to actually get involved, we?ve been tossing around the idea of adding additional capacity in our production run, and either offering you access to a portion of our orbiting spacecraft???or ??if there?s enough demand, actually build you an additional Space Telescope for your own use," Diamandis wrote. "We'd probably do this through a Kickstarter campaign, but ONLY if there's enough interest."

Among the ideas that Diamandis is floating:

$100 for a chance to direct the Arkyd-100 and take a high-resolution photograph of anyplace on Earth, or a celestial body.
A desktop-scale model of the Arkyd-100.
A half-day at the controls of a satellite, allowing you to take up to 50 photos from space.
Invitations to the Planetary Resources launch party.

The suggestion box (which also serves as a ballot box for the suggestions) takes the form of a Facebook-style comment section on Diamandis' Web posting. Thirty comments, plus comments on the comments, piled up in the first hour since Diamandis issued his invitation.

Planetary Resources co-founder Peter Diamandis invites private citizens to submit suggestions for potential Kickstarter space-telescope projects.

Planetary Resources isn't the only venture trying to take advantage of the crowdfunding model. Last week I wrote about the ArduSat project, which involves another guy named Peter (high-energy physicist and former Wall Street investment manager Peter Platzer). ArduSat's organizers are seeking $35,000 in Kickstarter pledges for the development of a sensor-laden nanosatellite that could be run as an orbital time-share. As of this writing, the pledge amount is at $31,631 with 24 days to go?? which means it's virtually certain ArduSat will hit its funding target.

I also mentioned the DreamUp project, which is offering space on the International Space Station's experimental racks for student-built experiments at rates as low as $15,500. DreamUp is a partnership involving NanoRacks and the Conrad Foundation, and has the added twist that American Express Membership Rewards points can be redeemed to cover the cost of flying an experiment.

Meanwhile, the nonprofit B612 Foundation says it's getting ready to unveil a privately funded, multimillion-dollar space telescope project to monitor the inner solar system for potentially threatening asteroids.

Egypt delays poll results, ousted president in coma

Egypt's election commission delayed the announcement of presidential results scheduled for Thursday, as tension spiked over who will succeed ailing ousted president Hosni Mubarak after moves by the ruling military to extend powers.

"Egypt's election commission, headed by Judge Faruq Sultan... has decided to delay the announcement of the presidential election run-off," the official MENA news agency said late on Wednesday, without giving a new date.

The run-off, which took place on June 16 and 17 pitted Muslim Brotherhood candidate Mohamed Morsi against Mubarak's last prime minister Ahmed Shafiq, with both candidates claiming victory.

The election commission said it was looking into appeals from lawyers of both candidates into alleged campaign violations and disputed vote counting.

The commission said it would "continue examining the appeals... which will require more time before the final results are announced."

The announcement came amid uncertainty over the health of Mubarak, following a flurry of reports about his condition.

Mubarak "is not clinically dead," a medical source told AFP. "He is in a coma and the doctors are trying to revive him."

"He has been placed on an artificial respirator," the source added, in an account confirmed by a member of Egypt's ruling military council, who spoke to AFP on condition of anonymity.

Egypt's state television carried a ticker item saying Mubarak was in "a coma and is not clinically dead."

State news agency MENA had earlier said the ousted strongman, 84, had been declared clinically dead after suffering a stroke in prison and being transferred to hospital.

"Hosni Mubarak is clinically dead," the report said. "Medical sources told MENA his heart had stopped beating and did not respond to defibrillation."

News of Mubarak's failing condition came amid a backdrop of legal and political chaos.

The Brotherhood appeared set on a confrontation path with the ruling Supreme Council of the Armed Forces (SCAF) which which issued a new constitutional declaration granting itself sweeping powers.

The state-owned daily Al-Ahram summed up the mood saying Egypt was facing "the most critical 48 hours in its history."

Early Monday the Brotherhood said their candidate had won the runoff, and on Tuesday provided what they said were certified copies of ballot tallies to bolster their claims.

But Morsi's rival Shafiq also claimed a victory, with his campaign accusing the Brotherhood of issuing false figures.

A group of independent judges -- headed by the ex-head of the Judges Union, Zakaria Abdel Aziz -- who monitored the voting process confirmed in a news conference that Morsi had won, according to their tally.

The new president, irrespective of the result, will not wield the near-absolute authority Mubarak enjoyed for three decades, after SCAF issued a constitutional declaration on Sunday claiming sweeping powers.

Mubarak's successor will also inherit a struggling economy, increased insecurity and the challenge of uniting a nation divided by the uprising and its deadly aftermath.

Should Mursi win, it will be a real test for the Brotherhood's ability to deal with problems on the ground.

The new dynamics will mean that "SCAF will command the national security of Egypt and leave domestic issues to the president. Any problems and the blame will be shifted to the elected representative," said Joshua Stacher, a political analyst and Egypt expert at Kent State University.

The SCAF's document said it would retake legislative powers from the Islamist-dominated parliament after the country's constitutional court ordered, last week, the body dissolved.

And it grants the military council veto power over the drafting of a permanent constitution, angering activists who denounced the declaration and an earlier order giving the army power to arrest civilians, as a "coup."

The Brotherhood insists the parliament retains legislative power and has pledged to counter the SCAF decisions with "popular activities."

On Tuesday night, the Brotherhood joined a mass demonstration in Cairo's Tahrir square, which attracted over 15,000 protesters, some celebrating Morsi's win as much as denouncing the military move.

The demonstrators were still in the square as the conflicting details of Mubarak's condition filtered in.

"It's divine retribution," said Saber Amr, a teacher. "God doesn't forgive those who do wrong to their people. God doesn't forgive those who kill innocents."

Nearby, Abdel Mottaleb, a Brotherhood supporter, offered a more conciliatory tone, saying "God will judge him."

Mubarak was taken to a Cairo prison on June 2, after a court handed down a life sentence against him over his involvement in the deaths of protesters during the 2011 uprising that pushed him from power.

His health deteriorated after the transfer, with doctors defibrillating him twice earlier this month, and reports saying he was suffering from bouts of depression, high blood pressure and shortness of breath.

NFL denies witness retractions in bounty probe

NEW ORLEANS (AP) ? The NFL denies accusations it covered up retractions made by key witnesses in its bounty investigation, or that Commissioner Roger Goodell has placed gag orders on Saints employees and others who could help punished players clear their names.

Lawyer Peter Ginsberg, who represents suspended Saints player Jonathan Vilma, made the accusations when punished players appeared earlier this week for an appeal hearing, a full transcript of which has been obtained by The Associated Press.

NFL spokesman Greg Aiello says claims of a gag order are "completely untrue," and that no potential witnesses were instructed by the league to stay away from Monday's hearing.

The transcript also shows that the NFL Players Association formally asked Goodell to recuse himself from ongoing appeal proceedings, contending that he is incapable of ruling "without the appearance of bias."

"The commissioner has publicly appeared on television and in other (forums) defending the discipline," NFLPA attorney Jeffrey Kessler said. "We would ask that the commissioner step down, to the extent that these proceedings continue, as the decision maker and that a neutral decision maker be appointed in his place."

The early portions of the transcript detail a series of verbal jousts that Ginsberg sought to deliver to the commissioner before leaving the appeal hearing early in protest.

In discussing witnesses he has been unable to speak with, Ginsberg told Goodell, "You have made threats to keep them from talking to us. You have refused to have them even participate in today's proceedings."

Ginsberg and the NFLPA had asked the NFL to compel several witnesses to appear at the appeal hearing, including Saints head coach Sean Payton, assistant head coach Joe Vitt, general manager Mickey Loomis, former defensive coordinator Gregg Williams, and former defensive assistant Mike Cerullo.

While the NFL declined to require anyone to attend the hearing, Aiello said the players and their representatives "were free to bring any witnesses of their choosing, including any coaches if they agreed to appear."

Ginsberg further asserted that Williams and Cerullo offered retractions of some of their initial statements to the NFL in the investigation, and that the league has neglected to share that information. Aiello denied such retractions were ever made.

Ginsberg also accused the NFL of distorting evidence introduced at the appeals hearing, including an email from marketing agent Mike Ornstein to Williams, who the NFL says ran a bounty program from 2009 to 2011.

In the email, Ornstein tells Williams he gave him "1,500 last week, I will give you another 1,500 the next four game (sic) and the final 2,000 the last 4." Ginsberg asserts that the NFL included the email as evidence even after Ornstein told Goodell the email related to a Gregg Williams charity.

The NFL declined to comment beyond its assertion in Monday's hearing that the total amount of money the email discusses ? $5,000 ? matches the amount listed next to Ornstein's name on a note outlining pledges made as "seed money" to a performance incentive pool that rewarded big plays as well as injurious hits.

Ginsberg told the commissioner he imposed a "misplaced punishment" which "cast a shadow not only on Mr. Vilma personally and professionally, but, I dare say, on the NFL and on the office of the commissioner in engaging in these proceedings."

He then concluded his statement by urging Goodell to "rescind any punishment against Mr. Vilma and to apologize in public for what you have done."

Not long after making those statements on the record, Ginsberg, who is also representing Vilma in a separate defamation lawsuit against Goodell in federal court in New Orleans, left the hearing with Vilma in protest, They did not return for an afternoon session.

Later in the hearing, Kessler said the remaining three punished players ? Saints defensive end Will Smith, Green Bay defensive lineman Anthony Hargrove, and linebacker Scott Fujita ? would listen to the remainder of the hearing, but would decline to participate because they believed the proceeding lacked elements required to meet the standard of a "fair hearing" under the NFL's current labor agreement.

"The essence of that hearing contains certain matters that are being denied to the players here," Kessler said. "In particular, we're not being given the opportunity to confront and cross-examine the actual witnesses who have evidence on this."

Kessler made an additional point that the NFL has already stated publicly that Williams, the defensive coordinator, ran the Saints' incentive pool, and that punishing employees for something their employers instructed them to do is "contrary to established legal principles."

The NFL continued its portion of the hearing in which it reviewed evidence against the players.

Goodell did not recuse himself and it did not appear he would consider doing so after league attorney Adolpho Birch responded to Kessler that an arbitrator already ruled Goodell has the authority to hear the appeal.
